iCare Collaborates with Casenet to Deliver Advanced Support for Home and Community-Based Services

Casenet, LLC, a leading provider of extensible care management solutions, today announced its collaboration with Independent Care Health Plan to deliver home and community-based services to its members using TruCare®. Enhanced coordination of care, including home and community-based services, is essential to reducing medical costs and delivering improved individual and population health. The AARP Public Policy Institute estimates that on average, Medicaid can provide home and community-based services to three people for the cost of serving one person in a nursing home1. The Casenet® TruCare solution delivers the only integrated care management platform with an embedded home and community-based services (HCBS) solution which supports the unique requirements of long-term care and waiver programs including those of Medicaid and other government-sponsored programs.

Independent Care Health Plan (iCare) has been using TruCare Home and Community Services to manage its members who prefer to live in a more familiar, independent setting while allowing treatment to be administered in a cost-effective manner. The iCare Family Care Partnership is a managed care program for the people of Wisconsin that fully integrates Medicare and Medicaid services, including long-term care services, and is designed to enable people with nursing home levels of needs to continue living in the community.

 

TruCare Home and Community Services enables coordinated, team-based management of long-term care and waiver programs including the selection and scheduling of home and community-based providers, authorizations and the costing of services. Personalized, member-centered care plans, including non-clinical, community-based services such as home meal programs and transportation are synchronized within the member profile and with all other care management activity, delivering a single platform for all care coordination needs. The resulting individual service plan (ISP) summarizes all of the activities related to the member’s care, including both clinical and home and community-based services, which can be brought together into a single document to be printed and used for member review and signoff.

 

The TruCare Home and Community Services module provides an efficient and effective way to manage the unique needs of members who need community-based care while also developing the person-centered care plans required by CMS. TruCare HCS enables care managers to:
